Email Authorization

Greenwood Medical Centre is happy to announce a trial of contacting the clinic via email. Please read our email policies below and ask at the front desk for an authorization form.  Failure to abide by this policy is grounds for immediate termination of your authorization to use this form of communication with our clinic.

Email Use Policies
Patients may use our clinic email address for the following purposes:
  • To book appointments
  • To reschedule, confirm or cancel appointments
  • To email the clinic with copies of documentation or forms not considered sensitive by the patient or physician (e.g. immunization records) in a manner similar to faxing
  • Allowing Greenwood Medical Centre to email patients regarding preventative care measures that are due or inform patients about clinic closures or schedule changes
  • Appointment time changes or cancellations
  • Notice of appointments outside our clinic with at least one week's notice
Patients need to agree NOT to use this form of communication for the following reasons:
  • For prescription refills
  • To request medical advice
  • To request test information or copies of medical documentation as this method of communication is not considered secure by the College of Physician and Surgeons of Ontario or the Ontario Medical Association

Please note offensive language will not be tolerated and may be grounds for immediate termination of the physician-patient relationship.

Please be aware that we will strive to treat any email correspondence as confidential medical informationHowever, we realize in this electronic era that such communication methods are far from secure and often not reliable.  If you do not get a response within a week of sending your email, please feel free to resend your communication or call us.
