Note to Patients Interested in Enrolment at Greenwood Medical Centre

We have paused the use of a clinic wait list in favour of the centralized Health Care Connect system, as advised by Ontario Health.   The clinic has kept the names on the list in the event that the process changes in the future.  However, we are currently only able to enroll unattached patients living in the Kingston East End catchment area who are directed to us by Health Care Connect.

Patients Living in Kingston East - Steps to Register

1. Register on Health Care Connect at https://hcc3.hcc.moh.gov.on.ca/ or call 1-800-445-1822.

2. Once Health Care Connect has notified you that you have been attached to Greenwood Medical Centre, please visit during an after hours clinic of your choice to verify your address and sign the roster form provided by our staff.  Clinic staff will require proof of address with a valid drivers license, purple Ontario ID card or 2 pieces of recent mail from a known company or government branch addressed to you (e.g. utility bill, health card renewal, etc.).  Addresses will be checked to ensure that patients reside within the catchment area.

3. Staff will ask you to acknowledge that you agree to all clinic policies.

4. Staff will ask you to complete the intake form linked below.

5. Greenwood Medical Centre will notify you that your file is active within 3 months.  Please do not book an appointment before this.  We will either schedule an initial in-person appointment (if required) or notify you that you may book appointments as you need.

6. Ensure that you are added to our secure email and online booking platform (Pomelo) after your first in-person appointment.  You can learn more about Pomelo on our Contact Us page.

Please complete this form only when directed to by staff after being attached to our clinic by Health Care Connect.  New patients will be required to show proof of address matching the clinic catchment area (see below).
