This is the website for the Greenwood Medical Centre in Kingston, Ontario.  We opened our doors in October 2010 and have five family physicians and nurses working with us.

Frontenac Lennox and Addington OHT Health Home Model 


In the past, individuals relied heavily on a single health-care provider.  If that provider departed or retired, they often found themselves without care. We are transitioning to a new model — Health Homes — where individuals are attached to a team of health-care providers.  This team-based approach ensures continuity of care, no matter what changes may occur with individual providers.  


Health Homes are being developed with a regional focus, meaning people can access team-based care close to their homes.  This approach is designed to provide convenient, accessible care within geographic boundaries, ensuring that health-care is integrated into the community it serves.


By working in geographical boundaries to provide care close to home, in team-based models that will attract new providers, we are optimistic that every part of KFLA will have a Health Home to connect people who are currently unattached.  In the last year, over 6,000 people have been attached to Health Homes in our region.

We have heard of some confusion about why people are being asked to both register on Health Care Connect, as well as attending special rostering events at primary care practices close to where they live.  In the past, people have been rostered to primary care only through Health Care Connect.  Now the FLA OHT, in collaboration with primary care providers and other partners, is trying to accelerate the process of attaching people to primary care providers close to where they live so that we can move more quickly toward our goal of attaching every person in our community to a primary care provider close to home. 


We understand the frustration that can come with waiting to be attached to a primary care provider.  Your patience is appreciated as we work hard to ensure everyone is connected to high quality team-based primary care.


The recent event held to roster new patients to the Greenwood Medical Centre’s East End Health Home marks the beginning of a broader, long-term effort to ensure that every resident in Kingston’s east end has access to primary care.  As capacity increases, Greenwood Medical Centre plans to expand access to more unattached residents in the area.


We encourage everyone without a primary care provider to register with Health Care Connect because it helps ensure that our region has accurate data on the number of people who are unattached.  This data is crucial for us to advocate for more resources and capacity to meet the needs of our community.  For more information about Health Care Connect, call 811 or 1-800-445-1822.



Frequently Asked Questions Regarding Attachment at Greenwood Medical Centre


What is being done to address the departure of a Greenwood Medical Centre family doctor in 2025?


We acknowledge the concerns raised by patients regarding the departure of a Greenwood Medical Centre family doctor in 2025.  Those patients whose physician is leaving the province have a provider until January.  In the meantime, the FLA OHT is continuing to build neighbourhood Health Homes.  Patients will be picked up for rostering in the Health Homes closest to where they live. 


What support is available for patients affected by their doctor's departure?


Other Health Homes are attaching people to care close to home.  Greater Napanee Health Home is attaching people within the Greater Napanee area and Midtown Kingston Health Home will be attaching 8,000 people when capacity allows.  Currently they are seeing anyone without a primary care provider.  Learn more about Greater Napanee Health Home and Midtown Kingston Health Home.  


Why did Greenwood Medical Centre hold a rostering event on August 17 to attach new people? 


The event to roster new patients to the Greenwood Medical Centre’s East End Health Home marks the beginning of a broader, long-term effort to ensure that every resident in Kingston’s east end has access to primary care.  Approximately 400 people, all of whom were geographically eligible attendees, were attached to a team-based primary care provider close to their home. Greenwood Medical Centre plans to attach 400 more people, including those from Health Care Connect, in the coming weeks and months.


Considering Greenwood Medical Centre’s goal is to attach new people, why doesn’t the Centre shift those patients to available spots?


Greenwood Medical Centre is building an East End Health Home that the FLA OHT is supporting. Those patients whose physician is leaving the province have a provider until January 2025. Previously, people were only attached to primary care providers through Health Care Connect. Now, to speed up the process, the FLA OHT in collaboration with primary care providers and other partners, is hosting special rostering events. These events allow us to attach people to providers close to where they live more quickly, while Health Care Connect continues to function as a parallel attachment stream.


Should I still register with Health Care Connect?


Yes, we encourage everyone without a primary care provider to register with Health Care Connect. Information Release in Regards to Dr Curry's Departure and Geographic Model of Health Care

 

We acknowledge the concerns raised by our patients regarding the recent changes at Greenwood Medical Centre, particularly in light of one family doctor's announcement of their upcoming departure in 2025. We understand the anxiety it has caused for those affected.


We have worked for over a year to recruit a replacement physician for this practice unsuccessfully. We continue to actively work to minimize any disruption in care for those patients and we will support their efforts in finding a new provider. For example, we are exploring options with other Health Homes in the area to see if they can assist in accommodating patients during this transition.


Prior to the announcement of this family doctor's departure, scheduled for 2025, Greenwood Medical Clinic was already in the process of transitioning to a regional Health Home serving a specific geographic area. This shift is part of a broader effort to enhance access to primary care across the region, ensuring that all residents receive comprehensive, coordinated care close to home.


At our recent rostering event, the goal was to attach more people to a primary care provider quickly, as we work to address the significant number of unattached people in the area. This is in addition to the Health Care Connect stream that has already attached 600 Patients to Greenwood Medical Centre earlier this year.


Approximately 400 people (100% of geographically eligible attendees) were attached to a team-based primary care provider close to their home on Saturday August 17, 2024.  Greenwood Medical Centre will attach 400 more people, including patients from Health Care Connect, in the coming weeks and months.


We remain committed to providing high-quality care and will continue to support our patients through this period of change and updates will be forthcoming. We appreciate the community’s understanding and patience as we work to ensure everyone has access to the care they need, close to home.
