Greenwood Medical Centre Catchment Change

The Greenwood Medical Centre catchment area has been expanded to include the area bounded by Kingston Mills Road east of Kingston Mills Locks / John F. Scott Road on the north, Joyceville Road / Howe Island Ferry Road on the east, the St. Lawrence River on the south and the Cataraqui River on the west.  It also continues to include Treasure Island and all residential streets off of Kingston Mills Road east of the locks.

The phone directory system at Greenwood Medical Centre has undergone some changes as of September 26, 2024.  These changes have been made to help the clinic manage a high volume of calls as our patient roster expands.  We recommend that patients who call the clinic please listen carefully to all menu options before making their selections.  Thank you for your patience while we implement these improvements to our services.

What is taking place to attach more people to a primary care provider in Kingston’s east end?


Greenwood Medical Centre, a partner in the Frontenac, Lennox and Addington Ontario Health Team (FLA OHT), is pleased to announce that by March 31, 2025, Greenwood will have rostered all individuals who reside in Kingston’s east end, are registered with Health Care Connect and do not currently have a primary care provider. 


What is a People-Centred Health Home and why are clinics like Greenwood Medical Centre rostering new patients within a specific geographic boundary?


Greenwood Medical Centre is expanding to become one of the first People-Centred Health Homes in the FLA OHT region. Health Homes are a new model of delivering team-based primary care in which people can access care close to where they live. The model is being adopted by FLA OHT partners like Greenwood Medical Centre to provide convenient, accessible care within geographic boundaries, ensuring that health care is integrated into the community it serves.  As this new model expands throughout the FLA OHT region, Health Homes will be the front door to the health-care system, including a team of health-care professionals dedicated to supporting peoples’ best health and wellness.  As capacity allows, new patients will be rostered to Health Homes based on the neighbourhood geographic boundaries that each Health Home serves. Existing patients who are already rostered to a primary care practice will remain with that practice as it develops into a People-Centred Health Home.  Greenwood Medical Centre began the process of implementing the People-Centred Health Home model and attaching new people from Kingston’s east end in April 2024, using two parallel intake processes: by rostering individuals registered on Health Care Connect and, to increase the number of people who could be rostered at one time, by holding a rostering event. Through this process to date, Greenwood Medical Centre has successfully attached approximately 900 new people living within Kingston’s east end who did not previously have a primary care provider. By holding periodic rostering events, while also rostering individuals registered on Health Care Connect as that system makes their names available, primary care clinics like Greenwood can attach more people more quickly. As capacity allows, other Health Homes in the FLA OHT region will also hold similar rostering events to attach people more quickly. 


Why did the patients of one physician at Greenwood Medical Centre receive notice they would be de-rostered from that physician’s practice at the same time as new patients were being rostered to other physicians at Greenwood?


Clinics like Greenwood Medical Centre provide the space and resources for physicians to care for their patients, but they are not involved in the opening, management, or closure of individual physician practices. Unfortunately, one of the physicians at Greenwood Medical Centre made the difficult decision to leave their practice. While this departure is unrelated to the clinic's expansion plans, it has understandably led to the misconception that some people are being de-rostered to make room for others.  That is not the case.  Any questions regarding the physician's departure should be addressed to the departing physician directly.


What’s next?


By working in geographical boundaries to provide care close to home, in team-based models that will attract new providers to this region, we are optimistic that every part of the counties of Frontenac, Lennox and Addington will have a People-Centred Health Home that cares for all individuals within the boundaries that it serves. In the past year, over 6,000 individuals have been successfully attached to People-Centred Health Homes in our region. Update Regarding Physician Departure

We appreciate you taking the time to read this update as Dr. Curry prepares to close his practice and move out of province.  


Our clinic has been doing our very best to help facilitate a transition of care to another provider/clinic.  While we remain committed to a geographic model of delivering care in an East End Health Home, we are unfortunately not able to assume all of Dr. Curry’s practice once he leaves. 


We would like to assure you that Dr. Curry has plans to continue your care until January 9, 2025.  We recommend you continue under his care until that time.  There are several clinics across our community that will hopefully soon be announcing capacity within their own regional geographic health homes.  We will post updates on our website as details become available. 


We have had multiple discussions with various levels of health administration to explore fair ways to help Dr. Curry’s patients access a new primary care provider.  These discussions have focused on ensuring that access to care is organized through a consistent and equitable process.  Based on this focus on equity, we will be asking all of Dr. Curry's patients to go through the same process of registering with Health Care Connect after January 9, 2025, to be linked to a new primary care provider, including patients who live within the clinic's geographic boundary (Middle Road on the north, Abbey Dawn Road on the east, the St. Lawrence River on the south, and the Cataraqui River on the west, plus Treasure Island).  


Greenwood Medical Centre is continuing to strive to provide care for all residents within our geographic boundaries who need a primary care practitioner.  We are confident that in the near future most areas in Kingston, Frontenac, Lennox and Addington will also have regional health homes with capacity to accept patients.  Please check our website periodically, as our boundaries may expand and we will also post any information that may be helpful to all residents of the Kingston and Napanee area.

We have heard of some confusion about why people are being asked to both register on Health Care Connect, as well as attending special rostering events at primary care practices close to where they live.  In the past, people have been rostered to primary care only through Health Care Connect.  Now the FLA OHT, in collaboration with primary care providers and other partners, is trying to accelerate the process of attaching people to primary care providers close to where they live so that we can move more quickly toward our goal of attaching every person in our community to a primary care provider close to home. 


We understand the frustration that can come with waiting to be attached to a primary care provider.  Your patience is appreciated as we work hard to ensure everyone is connected to high quality team-based primary care.


The recent event held to roster new patients to the Greenwood Medical Centre’s East End Health Home marks the beginning of a broader, long-term effort to ensure that every resident in Kingston’s east end has access to primary care.  As capacity increases, Greenwood Medical Centre plans to expand access to more unattached residents in the area.


We encourage everyone without a primary care provider to register with Health Care Connect because it helps ensure that our region has accurate data on the number of people who are unattached.  This data is crucial for us to advocate for more resources and capacity to meet the needs of our community.  For more information about Health Care Connect, call 811 or 1-800-445-1822.

In the past, individuals relied heavily on a single health-care provider.  If that provider departed or retired, they often found themselves without care. We are transitioning to a new model — Health Homes — where individuals are attached to a team of health-care providers.  This team-based approach ensures continuity of care, no matter what changes may occur with individual providers.  


Health Homes are being developed with a regional focus, meaning people can access team-based care close to their homes.  This approach is designed to provide convenient, accessible care within geographic boundaries, ensuring that health-care is integrated into the community it serves.


By working in geographical boundaries to provide care close to home, in team-based models that will attract new providers, we are optimistic that every part of KFLA will have a Health Home to connect people who are currently unattached.  In the last year, over 6,000 people have been attached to Health Homes in our region.

Information Release in Regards to Physician Departure and Geographic Model of Health Care

We acknowledge the concerns raised by our patients regarding the recent changes at Greenwood Medical Centre, particularly in light of one family doctor's announcement of their upcoming departure in 2025. We understand the anxiety it has caused for those affected.


We have worked for over a year to recruit a replacement physician for this practice unsuccessfully. We continue to actively work to minimize any disruption in care for those patients and we will support their efforts in finding a new provider. For example, we are exploring options with other Health Homes in the area to see if they can assist in accommodating patients during this transition.


Prior to the announcement of this family doctor's departure, scheduled for 2025, Greenwood Medical Clinic was already in the process of transitioning to a regional Health Home serving a specific geographic area. This shift is part of a broader effort to enhance access to primary care across the region, ensuring that all residents receive comprehensive, coordinated care close to home.


At our recent rostering event, the goal was to attach more people to a primary care provider quickly, as we work to address the significant number of unattached people in the area. This is in addition to the Health Care Connect stream that has already attached 600 Patients to Greenwood Medical Centre earlier this year.


Approximately 400 people (100% of geographically eligible attendees) were attached to a team-based primary care provider close to their home on Saturday August 17, 2024.  Greenwood Medical Centre will attach 400 more people, including patients from Health Care Connect, in the coming weeks and months.


We remain committed to providing high-quality care and will continue to support our patients through this period of change and updates will be forthcoming. We appreciate the community’s understanding and patience as we work to ensure everyone has access to the care they need, close to home.
