While OHIP covers most medical needs, there are services that we provide which are not covered.  Examples include, but are not limited to: 

1. Prescription renewals without an appointment

2. Sick notes

3. Referrals for physiotherapy, massage therapy, chiropractic, etc.

4. Forms (we aim to complete these within 60 days but please be aware that this can be upwards of 120 days due to extreme volume)

Please note that visits not medically necessary to complete forms are also uninsured and incur an additional fee that may or may not be included in the fee for the form.  

IMPORTANT: Listed form fees are estimates.  If the time required for the form to be completed by your provider is longer than estimated, the cost of the service may be adjusted to reflect this.
